It is always the right time to register your wishes for your health and end-of-life care. When you can no longer make health care choices for yourself, your Advance Care Plan will guide your designated Substitute Decision Maker in ensuring you receive the treatment and care you want.
We will explain the legal framework surrounding Substitute Decision-Makers, walking you through your options and recording your wishes. You will emerge from our discussion with a sharable document outlining your wishes should you no longer be able to speak for yourself.
Advance Care Plan conversations and document preparation require 90 minutes of your time and can be done virtually or in-person.
